Depending on your project, you may need to complete one or more of the following forms:
Letter of Authorization
Required if someone else is submitting the application on your behalf.
Commitment to General Review by Architect and Engineers
Needed for complex projects involving architects or engineers.
Schedule 1: Designer Information
To be completed by your project designer or anyone performing design activities.
Schedule 2: Septic System Design
Required for septic system applications, must be filled out by a qualified individual under the Ontario Building Code.
EEDS Prescriptive Path
For new homes or additions using the basic energy efficiency path under SB12.
EEDS Performance Path
For more advanced energy-efficient designs (e.g., Energy Star, EnerGuide) under SB12.
SB10 Form A and SB10 Form B
These forms apply to energy efficiency requirements for non-residential buildings.
Ontario Building Code Matrix
For industrial, commercial, and institutional (ICI) construction, to be completed by a qualified person.
